Experiencing problems in Dynamics 365 App for Outlook

Taken December 2016 update to our Dynamics CRM 2016 on-premise. I want to use dynamics 365 app for outlook. But having issue in authentication. I have set up server synchronisation for myself and this works successfully. I have deployed the App for Outlook to my Outlook 2016 application. When attempted to authenticate and provide email and password. It shows 404.
